What if I already have a primary care doctor?

Our team can coordinate the transfer of your medical records from any specialist or primary care physicians.

How is this different to other Concierge Medicine programs out there?

Why have a Concierge Doctor who isn’t a Hoag Doctor?
Do I have access to my doctor on the weekend?

Yes, Our physicians will be available to members by phone, text, and email. However, if it’s a medical emergency, patients are to dial 911. There may be times when the physicians recommend utilizing the expert staff at Hoag Hospital’s Emergency Department, an affiliated Hoag urgent care, or an alternative medical facility closest to your location. Our physicians and staff will always help coordinate care to ensure you are well taken care of 24/7.
Is this related to Hoag Executive Health?

Yes. Our Physicians are the Hoag Concierge and The Hoag Executive Health exam providers. The Executive Exam is designed as a one-time service for anyone looking for a comprehensive, personalized, in-depth analysis performed in one day. Some patients begin by participating in the Executive Exam and then decide to join our Concierge membership and select us as their primary care provider for all their medical needs. Contact us to learn more about the Executive Health Exam. Ask about our bundled service pricing for both services.

Does Insurance Cover My Membership?

I Have a PPO, Medicare or EPO Plan
Great! Hoag Concierge Medicine accepts PPO, Medicare and most EPO insurance plans.
- We will bill your insurance for office visits and procedures, just as a traditional health care provider.
- Your insurance benefits and co-pays, deductibles and non-reimbursable fees still apply as specified by your PPO insurance plan.
- A monthly membership fee still applies.
Please contact us to confirm if we are contracted with your specific Insurance provider.
I Have an HMO Plan
Unfortunately, Hoag Concierge Medicine does not participate in any HMO plans.
- We will bill you directly for office visits and procedures
- A monthly membership fee still applies.
If you thinking about switching to a PPO plan during your next Open Enrollment Period, you may still schedule a consultation to see if our membership fits your goals and needs.

If you are still interested in our Comprehensive Physical, are unable to switch from your HMO plan, and want to remain with your existing primary care provider, considering our Hoag Executive Physical or 360 Wellness Exam.
My Employer Offers Reimbursement

Great! Consider our Hoag Executive Physical, the best option for those with employer reimbursement. All participants of the physical can upgrade to a Concierge Membership. Speak with our team to find the best options for your situation.
